MIST Code of Conduct Agreement Form (MUST FILL OUT ASAP)

Every competitor MUST fill out this Code of Conduct and waiver form in order to attend MIST. Some info you will need as you complete it (takes only a minute):

Once you complete that form, please check "Yes" on this form so that we can check off who has filled it out and who has not! If you don't check "Yes" on this form after submitting the waiver, we have no way of knowing whether or not you did it and we will annoy you with emails so please don't forget!

Don't forget that 4-week early submissions are due this Friday and 2-week ones are due soon! (See attachment for details)
